Full-spectrum care for patients and families.
From managing pain and symptoms to supporting your family through grief, we provide compassionate care at every stage.
Comfort-focused care for the final chapter.
Hospice care is for patients with a terminal diagnosis who choose to focus on comfort and quality of life rather than curative treatment. It's not giving up — it's choosing to live fully on your own terms.
Medicare and Medicaid cover hospice for patients with a life expectancy of 6 months or less. Coverage can extend in 60-day increments as long as the patient continues to qualify.
What's included:

Relief from pain and symptoms, at any stage.
Palliative care is specialized medical care focused on providing relief from pain, symptoms, and the stress of a serious illness. Unlike hospice, palliative care can be provided alongside curative treatment — at any age, at any stage.
Our interdisciplinary team works with your existing physicians to provide an extra layer of support — managing symptoms, navigating difficult decisions, and ensuring your wishes are honored.
You don't have to grieve alone.
Grief is not a problem to be solved — it's a natural response to love and loss. Our bereavement team provides compassionate support for families before, during, and after the loss of a loved one.
Adult Grief Support Groups
10-week workshops in a warm, safe group setting. Starting February 2026.
Heartstone Youth Grief Program
Creative grief support for children and adolescents, September through May. Pre-registration required.
Individual Bereavement Support
One-on-one support from our trained bereavement counselors.
